Description

If a major workplace injury or illness occurred, what would you do?  How would you properly investigate it? What could be done to prevent it from happening again?  A properly executed accident/incident investigation drives to the root causes of the workplace accident to prevent a repeat occurrence. A good accident/incident investigation process includes identifying the investigation team, establishing/reviewing written procedures, identifying root causes, and tracking all safety hazards found to completion. This webinar course will cover all of the steps necessary in identifying how to properly conduct a workplace accident/incident investigation to drive to the root cause.

Learning Objectives:-

When investigating accidents, the more details the better.  You should also investigate as soon as you can, so conditions don’t change. A complete accident investigation determines the following:

  • What happened?
  • When did it happen?
  • Where did it happen?
  • Who was involved?
  • Why did it happen?
  • How can it be prevented from happening again?
  • The observations of co-workers can be critical, and if you are interviewed, heed the following:
  • Make mental or written notes about the accident before the investigation starts.
  • Avoid talking to others before talking to the investigator, since this may confuse the facts.
  • Answer all questions about the incident as accurately as possible.
  • Don’t change anything at the scene of the accident.

Areas Covered:-

  • How to properly investigate accidents/incidents to determine the root cause.
  • How to define corrective action plans directed at preventing a repeat occurrence of an accident/incident.
  • What are the elements of a good accident/incident investigation?
  • Having accurate and complete information.
  • Completing a description of the sequence of events.
  • Identification of Factors Contributing to the Accident.
  • Identification of root causes.
  • Identify corrective actions to address Root Causes.
  • Timeliness of completing of accident/incident investigation.
  • Discuss Root Cause Analysis Tools, including Fish Bone Diagram and 5-Whys.
  • Answer site-specific questions and concerns about accident/incident investigation.
